Before you can leave for camp we need a completed Consent to Treat Form. While we hope we never have to use it, we cannot let anyone attend Eagle Camp without this form on file.

If you feel you cannot attend Eagle Camp due to the cost, let us know. For those that fill out the form and are found to be in need, we can waive the registration fee.
